The death of Jeffrey Epstein continues to draw scrutiny years after he was found dead in a Manhattan jail cell. A newly released transcript has revealed that the doctor who conducted Epstein’s autopsy initially hesitated to declare his death a suicide.
Epstein, who had been charged with sex trafficking, died on August 10, 2019 while awaiting trial at a federal detention facility in New York. His death immediately triggered widespread speculation, partly because of his powerful connections and the serious allegations he faced.
The new details show that uncertainty existed even within the official autopsy process before a final ruling was issued.
Autopsy Initially Listed as “Pending Studies”
The autopsy was conducted on August 11, 2019 by Kristin Roman from the New York City medical examiner’s office. According to the transcript, Roman did not immediately classify the manner of death. Instead of marking the case as either homicide or suicide, she checked a third option on the death certificate: “pending studies.”
Roman later explained to investigators that she chose this option because of the unusual circumstances surrounding the case and the intense public attention it was receiving. She said she wanted to ensure every detail was carefully reviewed before making a final determination.
Her hesitation reflected the seriousness of the situation. Epstein was one of the most high-profile inmates in federal custody at the time of his death.
Final Decision Came From Chief Medical Examiner
The official ruling came five days later. On August 16, 2019, Barbara Sampson, who led the office at the time, formally ruled Epstein’s death a suicide by hanging.
Roman stated that her earlier hesitation was not because she believed another cause was likely, but because she wanted to be thorough. She told investigators that she was “not as convinced as I would like to be” about certain details when the autopsy was first completed.
Even so, she ultimately agreed with the final conclusion reached by her office.

Bone Fractures Remain a Point of Debate
One detail from the autopsy continues to attract attention. Roman reported fractures to Epstein’s hyoid bone and thyroid cartilage in his neck. According to her findings, these injuries were consistent with hanging.
However, the conclusion has been challenged by Michael Baden, who was hired as an observer by Epstein’s legal team. Baden argued that the pattern of fractures was unusual in suicidal hangings.
He stated he had rarely seen three fractures of that type in cases involving suicide and suggested they could be more consistent with homicide. The disagreement between the two medical professionals has helped keep the debate alive.
Questions About the Noose and Cell Scene
Roman also acknowledged uncertainty about one specific element of the case: which noose was responsible for Epstein’s death. Investigators recovered materials in the jail cell believed to have been used in the hanging. Roman said she was not entirely certain which of those items caused the fatal injuries.
Despite that uncertainty, she maintained that it did not change her overall conclusion about the cause of death.
Epstein’s Death Continues to Fuel Suspicion
Epstein’s death occurred while he was being held at a federal detention center in Manhattan, where he was awaiting trial on charges related to a large sex trafficking operation. Because of the seriousness of those charges and the number of powerful individuals connected to him, the circumstances of his death quickly sparked intense speculation.
Security failures at the jail — including broken cameras and guards who reportedly fell asleep — added to public skepticism about the official narrative.
Why the New Transcript Matters
The newly released transcript does not overturn the official ruling of suicide. However, it provides a clearer look at the internal decision-making process within the medical examiner’s office. Roman’s initial hesitation shows that the determination was not made instantly and that questions were examined before a final ruling was issued.
For critics of the official explanation, the document adds another layer to the ongoing debate. For others, it demonstrates that the investigation followed a methodical process despite the intense scrutiny surrounding the case.
Years after Epstein’s death, the circumstances surrounding his final hours remain one of the most controversial moments in modern criminal investigations.
