Emma Heming Willis has opened up about the guilt she felt while deciding whether to celebrate her 50th birthday as her husband, Bruce Willis, continues living with frontotemporal dementia.
During a conversation with Hoda Kotb, Heming Willis admitted that she had not felt particularly celebratory and initially considered allowing the milestone to pass without a party.
A friend repeatedly encouraged her to reconsider, prompting Heming Willis to ask what Bruce would have wanted her to do. She concluded that her husband, who loved large celebrations, would have wanted her to enjoy the occasion with family and friends.
Heming Willis ultimately marked her birthday with tacos, margaritas and chocolate cake, but said the decision reflected an ongoing struggle with caregiver guilt.
She Initially Considered Skipping the Celebration
Heming Willis discussed the decision on the August 5th episode of Making Space with Hoda Kotb. “I have not really been in the celebratory feelings of late,” she said. “I was unsure if I really wanted to do anything.”
A friend continued reminding her that she might regret failing to recognize such a significant birthday. “You don’t wanna miss out on your 50th,” Heming Willis recalled being told. “You have to celebrate in some form or fashion.”
After considering that advice, she decided she did not want to look back and question why she had allowed the milestone to pass unnoticed.
Caregiver Guilt Made the Decision More Complicated
Heming Willis told Kotb that guilt has remained a recurring part of her experience caring for Bruce. “I always wrestle with it,” she said. “I think guilt is something that I am always carrying, but I’ve learned that it is really not helpful.”
She eventually returned to one question: “What would my husband want for me?” Heming Willis said Bruce would have wanted her to celebrate her life with the people closest to her. “He would want me to have a big, fun bash,” she said. “He would want me to celebrate my life with my friends and family.”
She Celebrated With Tacos, Margaritas and Chocolate Cake

Heming Willis held the party at home on June 13th, five days before her 50th birthday. When friends asked what she wanted, her answer was simple: tacos, margaritas and time with the people who had supported her through different stages of her life.
The menu also included her preferred chocolate cake, not an elaborate dessert, but what she described in her Instagram caption as “the fluffy Duncan Hines kind.” Heming Willis called the gathering a “lovely, intimate night.”
She also noted that the New York Knicks won that evening, pleasing a guest list she estimated was 98% Knicks fans. “It was a great celebration,” she told Kotb.
Heming Willis Was Ready to Leave Her ‘Heavy’ 40s Behind
Bruce retired from acting in 2022 after his family announced that he had been diagnosed with aphasia. In February 2023, the family disclosed that his condition had progressed and that doctors had given him the more specific diagnosis of frontotemporal dementia. Heming Willis has since spoken publicly about caregiving, the isolation families can experience and the need for greater education and support surrounding FTD.
On her actual birthday, June 18th, she shared an older video of Bruce using helium from a balloon before singing “Happy Birthday” to her in a high-pitched voice as their daughters laughed nearby. “My 40s were heavy, yet I’m proud of how far I’ve come as a wife, mom, care partner, and advocate,” she wrote. “I’m also proud of the work we’re doing through the Emma & Bruce Willis Fund and what lies ahead.”
